 How will you send the voided sample to us? If you have ordered this product from this account from ASAP before and the bank has not changed, you do not need to send a sample; simply choose Reorder. Otherwise specify how you will send it (your order will be on hold until we receive the sample). If this is a new account, a starter cheque or specification sheet from your bank can be sent. |

 Print a custom logo? If ASAP has already set up your logo for this form, choose Print Existing Custom Logo and enter the logo code below from your reorder form. If you are sending us your logo for the first time, we will need camera ready artwork or a .bmp, .jpg or .gif file. Note that logos are currently printed in black & white (or gray). |

 If you are reordering, type the "Account Code" found on your reorder form from us (included near the end of your last order; if you cannot locate it, then type "Reorder" and rush us a voided sample from your current order). Then in the "How will you send your sample to us" field above, choose Reorder. If you require any changes from your last order, type them in the Notes area below. |
