How will you send the voided sample to us? If you have ordered this product from this account from ASAP before and the bank has not changed, you do not need to send a sample; simply choose Reorder. Otherwise specify how you will send it (your order will be on hold until we receive the sample). If this is a new account, a starter cheque or specification sheet from your bank can be sent.
If you are reordering, type the "Account Code" found on your reorder form from us (included near the end of your last order; if you cannot locate it, then type "Reorder" and rush us a voided sample from your current order). Then in the "How will you send your sample to us" field above, choose Reorder. If you require any changes from your last order, type them in the Notes area below.
Account Name Especially when you have more than one bank account, providing a name to refer to this to account will make it easier to refer to the right account when reordering or calling our customer service staff (eg. Operating Account; Payroll Account; Joint Account; etc.)

For security and accuracy, if this is the first time you have ordered deposit slips for this account from us, we will ask you send (eg. fax, mail, etc.) us a voided deposit slip from this account. This is where we will get the information that will be printed on your deposit slip (your name and address, bank name and address, MICR transit and account information). If this is a new account, a starter deposit slip or MICR specification sheet can be substituted (either handwrite how you want your name and address to appear, or enter it into the Notes section above). If you want changes to how your name and address are currently printed, handwrite the changes onto the sample or enter it into the Notes section above.
